    Job Summary We are seeking a dedicated and knowledgeable Referral Navigator to join our healthcare team. The ideal candidate will play a critical role in ensuring that patients receive timely specialty medical care and testing not provided by our Center. This role involves face-to-face interactions with patients to coordinate appointments, dates, and financing for specialty services deemed necessary by medical providers.

    Duties

    Collaborate daily with patients to schedule specialty appointments by contacting providers, coordinating dates, and arranging financing options.

    Send medical records, referral forms, and orders to specialty providers to ensure accurate and timely documentation of patient history, medications, and reasons for referral.

    Educate patients on testing procedures such as CT scans, X-rays, and MRIs.

    Track referrals and appointments through the Electronic Medical Record (EMR) system, maintaining accurate records.

    Partner with Case Managers to address patient needs such as travel, counseling, and other support services.

    Support the Patient-Centered Medical Home (PCMH) model by developing and adhering to policies and procedures that promote patient education and engagement.

    Provide individualized education to empower patients to effectively manage their health.

    Participate in required organizational training, including PHCC, OSHA, and Emergency Preparedness.

    Follow up with external providers to ensure patient compliance with scheduled appointments.

    Assist providers in meeting meaningful use criteria.

    Receive and track all testing results.

    Exhibit initiative, collaboration, and professional judgment to effectively carry out additional duties as assigned.

    Experience

    Strong interpersonal and communication skills for patient interaction and coordination with specialty providers.

    Proficiency with EMR systems and maintaining detailed records.

    Knowledge of medical terminology and testing procedures is preferred.
