    RESPONSIBILITIES

    Exceptional Customer Service

    Ability to communicate and work effectively with people of varying backgrounds, health care professionals and support staff at all levels.

    Must have respect and regard for the dignity of all patients and their families / caregivers.

    EHR Inbox Management

    Process inbox messages in a timely manner

    Phone Messages / Telephone Encounters

    Return messages and relay pertinent information in a timely manner to patients or other entities as needed.

    Visit Preparation

    Preparing patients for medical procedures and assist with procedures.

    Cleaning and Sanitizing rooms after each patient

    Patient Care within Scope of Practice

    Conduct a comprehensive assessment of the patient’s health and psychosocial needs and develop a treatment plan collaboratively with the patient (and family or caregiver) and health care team.

    Record all interventions – phone, email, in-person – in patient chart. Compile and produce reports on service delivery and outcomes.

    Work with RN Care Team Director and Clinical Operations Director to obtain data needed for outcome measurement reporting.

    Report patient progress in patient files and to medical staff.

    Perform basic laboratory and point of care testing.

    Maintain patient’s medical records.

    Work with provider(s) to send medication refills to appropriate pharmacy.

    Work with patients - new-born to geriatric, women’s health, OB, immunizations, and more

    Communicate relevant information to the various departments to maintain continuity of care.

    Stay up to date with immunization changes

    able to obtain Vital Signs (Manually Blood Pressure)

    Able to obtain Screenings/Measurements

    Provide Patient Education as needed

    Physical Requirements:

    While performing the duties of this job, employees are regularly required to sit, walk and stand; talk and hear, both in person and by telephone. Team members will perform their duties while standing for extensive periods of time. Good eyesight and manual dexterity skills are essential. Must have strength sufficient to assist with lifting and transferring a patient and perform CPR. Must have sufficient mobility to bend, stoop, and bend down to the floor; ability to move around rapidly; and to move in small, confined areas. May be exposed to infectious diseases, blood-borne pathogens, and occupational dangers.
