    Job Summary

    We are seeking a Registered Nurse Case Manager to join our team. The Registered Nurse Case Manager is a member of a multidisciplinary team including a social worker, clinical coordinator, community care aides, and staff, who provides comprehensive intake, assessment, discharge planning, medical case management, and non-medical case management services to all members of the Joseph’s House Community. The Registered Nurse Case Manager works directly with leadership, staff, residents, community friends, and community partners.

    Responsibilities

    Triage and admission of new residents

    Oversight of resident care, including symptom and medication management, and the provision of daily personal care

    Developing and maintaining resident care protocols

    Case management of residents either for end-of-life care, or medical stabilization and transition back to the community

    Close coordination of care with outside providers (outpatient hospices, hospitals, outpatient clinics) and advocacy on behalf of residents within the healthcare system as needed

    Training and supervision of resident care staff and volunteers

    As needed, accompaniment to medical appointments and

    Shared weekend and overnight on-call.

    Qualifications

    Currently licensed Registered Nurse (RN) in District of Columbia

    Five plus years of Registered Nurse experience is required

    Past experience working with vulnerable populations, and a desire to bring one’s whole self to the service of our community

    An ability to remain peaceful and productive in times of stress, and to collaborate effectively in a multicultural community

    Additional Requirements

    Ability to drive a vehicle

    Valid and unrestricted driver’s license

    Proof of vaccination against COVID-19

    Valid driver’s license
