We have a unique and interesting position in the East Valley perfect for a retired RN/LPN looking to work part time (approximately 20 hours per week) to provide one on one oversight with their clients while educating our caregivers. RNs/LPNs that work with ComForCare are ideally those that love working with people and enjoy a regular change of scenery as opposed to an institutional-style setting. RNs/LPNs generally do not have to participate in many ""front line" nursing activities, acting mainly as a position of authority and performing administrative-style duties.

What you’ll be doing:
Initial Client Home Evaluations
Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
Develop appropriate patient-centered care plans to insure best possible home care
Council clients and their families regarding self-care, chronic condition management, and
Evaluate clients' responses to care and provide interventions with a focus on achieving realistic client outcomes.
Reporting changes to update the plan of care to reflect progress towards goals and outcomes.
Maintaining client records showing systematic assessments, planning interventions, and evaluations.
Initiating referrals as needed to other health team members.
Hospital Relations
Act as the clinical care coordinator for clients and their families, collaborating with other health care professionals as needed.
Initiate referrals as necessary
Accompany marketer to conferences and other events to educate the medical community
Working with Caregivers
Works with the office team to determine a caregiver with appropriate personality and skill set for the client.
Review care plans with the caregivers to ensure understanding that helps clients receive the best care possible.
Help with training and instruction for caregivers on critical skills that may be needed when assisting with care.
Participate in caregiver education and training sessions.
What we’re looking for:
Graduate of an accredited school of professional nursing.
Current license as a Registered Nurse (RN) or LPN.
Adaptable to work a flexible schedule.
Highly competent to evaluate situations quickly and concisely.
Competent and capable of providing accurate information for best practices for client health and condition management.
Current CPR and First Aid certification.
Access to reliable transportation in order to visit with clients in their homes.
Able to travel 20-30 miles per day.
Maintains confidentiality in all aspects of the job.
Knowledge of safety and infection control practices.
