  • Responsibilities

    RN Case Manager (Homecare) position available Full-time role, 40 hours per week Typical hours: 8:00 am - 4:30 pm, Monday to Friday Weekend and holiday rotation required, typically every 4th or 5th weekend and holiday RN Care Center - PT Day position available Part-time role, 12-hour shifts from 6 am to 6:30 pm Work every third weekend with the opportunity to pick up additional shifts RN Care Center - PT Night position available Part-time role, 12-hour shifts from 6 pm to 6:30 am Work every third weekend with the opportunity to pick up additional shifts Responsibilities: • Assess patient symptoms thoroughly and provide advanced care management accordingly. • Coordinate interdisciplinary patient meetings to ensure a smooth continuum of care. • Supervise various staff members, including volunteers, hospice aides, and primary caregivers. • Educate patients and their families on the intricacies of insurance benefits. • Secure necessary medical orders from Physicians and Medical Directors. • Conduct initial nursing assessments and arrange for essential medical equipment. • Keep abreast of all job-related protocols and obligations. • Fulfill any additional duties as assigned. Qualifications: • Must be a graduate of an approved School of Nursing. • Should possess a current RN licensure in Ohio and eligibility for Indiana licensure, or vice versa. • Requires at least two years of experience in Medical/Surgical, Oncology, or community health nursing. • Preferably, should have previous experience in hospice care.
