    Overview: We are a leader in community healthcare for over a century, specializing in providing care for vulnerable populations including at-risk children, the elderly, those with chronic illnesses, and individuals facing end-of-life care. We are currently seeking a full-time Home Health RN in Middlesex County, NJ. The ideal candidate will deliver skilled nursing care, demonstrate effective communication and collaboration, and prioritize patient safety. This role requires strong organizational skills, independence, and a commitment to confidentiality. Candidates should have experience in home health care, and the position will cover Middlesex County, NJ.

    Qualifications:

    Required:

    Associate degree or higher

    Valid Registered Nurse (RN) license in New Jersey (NJ)

    Valid American Heart Association Basic Life Support (AHA BLS) certification

    1+ years of experience in any of the following: Wound Care, Burn Unit/Wound Clinic

    1+ years of experience in both Medical-Surgical (Med-Surg) and Home Health Care

    Preferred:

    Bilingual in Spanish

    RN License Number

    Specialized certifications

    Key Responsibilities:

    Provide direct or indirect skilled comprehensive nursing care to patients across various age and diagnostic groups

    Utilize the nursing process to collect data, plan, and evaluate nursing care

    Implement preventive, therapeutic, and rehabilitative nursing care based on patient needs

    Communicate effectively with healthcare providers, patients, and families

    Maintain awareness of patient comfort and safety needs

    Record significant patient conditions and reactions, notifying supervisors or physicians as necessary

    Respond to life-saving situations following nursing standards and protocols

    Convey pertinent information to the healthcare team through oral and written communication

    Schedule and Shift Details:

    8:30 AM - 4:30 PM, with one weekend per month

    Includes working one summer and one winter holiday
