Job Title: Home Health Care Registered Nurse
Reports to: Director of Nursing or Case Manager
Summary:
We are seeking a compassionate and experienced Registered Nurse to join our Home Health Care team. As a Home Health Care Registered Nurse, you will provide high-quality, patient-centered care to patients in their homes, focusing on improving health outcomes, reducing hospital readmissions, and enhancing patient satisfaction.
Responsibilities:
. Conduct initial and ongoing assessments of patients' physical, emotional, and social needs.
. Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
. Provide skilled nursing care, including wound care, medication management, and education.
. Monitor patients' progress, identify potential complications, and take prompt action to address them.
. Educate patients and families on disease management, treatment options, and self-care techniques.
. Coordinate care with other healthcare professionals, including physicians, therapists, and social workers.
. Document patient information accurately and timely, maintaining confidentiality and HIPAA compliance.
. Participate in case conferences, quality improvement initiatives, and team meetings.
. Stay up-to-date with industry developments, best practices, and regulatory requirements.
. Collaborate with the interdisciplinary team to achieve patient goals and optimize outcomes.
Requirements:
. Current RN licensure in the state of practice.
. Minimum 1-2 years of experience in home health care, hospice, or acute care.
. Strong clinical skills, critical thinking, and problem-solving abilities.
. Excellent communication, interpersonal, and patient education skills.
. Ability to work independently and as part of a team.. . Familiarity with electronic medical records (EMRs) and mobile documentation systems.
. Reliable transportation and ability to travel to patient homes.
Preferred Qualifications:
. Home health care experience or certification (e.g., HHC RN, OASIS).
. Knowledge of Medicare regulations and guidelines.
. Experience with wound care, chronic disease management, or palliative care.
