    Reports to: Director of Nursing as it relates to the information needed. Also, to Medical Director as it relates to the treatment of patients in Life Care Inc. programs.

    Workers Supervised: None

    Interrelationship: Must communicate all pertinent information to Director of Nursing, Medical Director, Clinical Director, and additional staff.

    Primary Job Responsibilities:

    Actively participates in working towards the organizational and departmental goals for providing exceptional patient experiences.

    Collaborates with patient and healthcare teams to assess, plan, implement, and/or evaluate patient's plan of care.

    Assures quality of care by adhering to industry standards and measuring health outcomes against patient care goals and benchmarks.

    Develops and implements educational plans and/or goals to assist the patient/client in achieving maximum health care benefits.

    Promotes and ensures patient/client safety by implementing established nursing care protocols, adhering to regulatory standards, and overseeing medication management, including storage, reconciliation, administration, and compliance monitoring.

    Maintains a safe and secure environment while prioritizing patient safety by adhering to agency policies and regulations, while demonstrating proficiency in all patient care tasks. Assesses the psychological, physiological, sociological, cultural, nutritional, and spiritual needs of patients/clients through comprehensive interviews with various stakeholders including patient, family, case manager, and/or designated power of attorney.

    Promptly identifies and reports any changes in patient/client health status, documenting observations and adjusting care goals within a 24-hour timeframe.

    Remains up to date on state and federal regulations pertinent to designated nursing practice areas, such as behavioral/mental health, developmental disabilities, youth services, and home health care.

    Documents nursing assessments and interventions in the electronic health record within 24 hours of patient/client interaction, ensuring accuracy and compliance.

    Offers follow-up and guidance to patients/clients regarding their healthcare inquiries and issues.

    Administers medications and treatments as directed by Physicians and/or Nurse Practitioners, promptly documenting all actions taken.

    Provides medical/nursing updates to Physicians and/or Nurse Practitioners regarding patient/client responses to medications and treatments, including any reported side effects.

    Advocates for patients/clients by promptly addressing and following up on healthcare concerns and issues.

    Maintains and enforces HIPAA compliance/Privacy laws.

    Other related duties and responsibilities

    Qualifications:

    RN Licensure with reciprocity in the District of Columbia

    BLS for the Healthcare Provider

    Behavioral health nursing experience preferred in an acute care setting.
