Registered Nurse (RN) for Home Health Agency (San Antonio)

We are a family-owned Home Health Agency with over 20 years of experience in providing exceptional care. As we continue to expand, we are seeking dedicated and skilled Registered Nurses to join our team. We offer both full-time and part-time positions, with competitive salaries. Bilingual candidates are preferred but not required.

Job Summary: We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient’s best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.

Responsibilities:

Recertifying and admitting patients

Providing high-quality care to our patients

Traveling within the San Antonio area for patient visits (mileage compensation provided)

Coordinating with other colleagues to assess, plan, implement, or evaluate patient care plans

Administering medications and monitoring patients for side effects or reactions

Documenting any changes in patient conditions and reporting on changes

Maintaining accurate records of patient care and discussing observations with the supervisor

Requirements:

Currently licensed Registered Nurse (RN) in the state of employment

Minimum of one year of experience working with Medicare and Medicaid patients

Reliable transportation for required travel within the San Antonio area (mileage compensation provided)

Valid driver’s license

First aid/CPR certification required

Excellent interpersonal and client care skills

A motivated and compassionate individual ready to improve the lives of our patients
