    Position Overview :

    Title: Rehabilitation Specialist

    Department: Social Rehabilitation (Oasis/AXS)

    Classification: Non-Exempt; Union

    Schedule: Monday-Friday, 8am-4pm (1st shift)

    The Rehabilitation Specialist provides wrap-around, all-inclusive, social rehabilitation services for all Oasis and AXS members, to foster growth and independence for daily living.

    Essential Functions:

    • Facilitate virtual, office based and community educational group activities.

    • Utilize various virtual platforms including Microsoft Teams for employee communication and GoToMeeting platforms for Person Served (PS) group meetings.

    • Instruct members (PS) in all areas within the AXS/Oasis Center club houses.

    • Work with community-based resources/agencies to ensure availability and access to resources for participating PS.

    • Assists in coordination and implementation of new programs.

    • Provides opportunities by implementing measurement tools and opportunities to monitor and improve consumer satisfaction.

    • Attends monthly team meetings and supervision with the Rehabilitation Services Program Coordinator II, as scheduled.

    • Become knowledgeable of the various Clubhouse models to garner greater insight into social rehabilitation theories and practices.

    • Acts as case coordinator for assigned clients including but not limited to interviewing PS, formulating recovery plans to include goal setting and implementation completes current and thorough documentation, (i.e. recovery plan, releases of information, in addition to quarterly and monthly contacts).

    • Develops a supportive atmosphere to maximize PS growth and recovery.

    • Provides safe transportation to PS as needed in agency vehicles.

    • Obtain/maintains Material Safety Data Sheets (MSDS).

    • Reports progress and concerns of PS to employees and outside agencies; complies with the Psychiatric Security Review Board (PSRB) reporting protocols.

    • Outreach with other agencies and service providers within and outside of the network to foster a larger sense of community

    • Adheres to all agency policies and procedures.

    • Provide programmatic coverage as needed.

    • Provide monthly outreach phone calls to Person Served to assist in increasing attendance, problem solving barriers to attending, and supporting the team in meeting goals

    • Monitor and facilitate use of OASIS/AXS space and property.

    • Administer first aid according to prescribed procedures and notify emergency medical personnel when necessary.

    • Organize, lead, and promote activities at the OASIS/AXS Centers and within the community.

    • Greet new arrivals to activities, introducing them to other participants, explaining facility rules, and encouraging participation.

    • Confer with management to discuss and resolve participant complaints.

    • Explain safety procedures to participants in recreational activities and demonstrate use of materials and equipment.

    • Complete and maintain time and client attendance forms.

    • Evaluate services to determine if they are producing desired results.

    • Ascertain and interpret group interests, evaluate equipment and facilities, and adapt activities to meet participant needs, fostering the clubhouse model.

    • Encourage participants to develop their own activities and leadership skills through group discussions.

    • Direct special activities or events such as performing arts and art exhibitions.

    • Schedule maintenance and use of facilities.

    • Take clients on community outings.

    • Provide for entertainment and set up related decorations and equipment.

    • Assist with the maintenance and upkeep of OASIS/AXS facilities and areas.

    • Attend all team meetings and trainings as assigned.

    • Other duties as may be assigned.

    Minimum Qualifications:

    • High School diploma (or equivalent) required; an Associate’s degree, or higher, in a related field, is preferred
    • A minimum of one (1) year of behavioral health/human services related experience required
    • Previous experience must include working with individuals who have a mental health diagnosis and/or substance use (experience with 18-25 year old population served preferred)
    • Custodial care will not be counted as experience
    • Excellent written and oral communication skills required
    • Must be competent user of technology including e-mail, agency intranet, and the electronic client record
    • Current valid driver’s license required
    • Candidate must be CPR/First Aid certified or eligible

    Physical Requirements:

    • Regularly required to sit, use hands to finger, handle or feel objects, tools, or controls, and climb stairs
    • Regularly required to walk, talk and/or hear
    • Speaking and hearing ability should be sufficient to communicate effectively by phone or in person at normal volumes
    • Vision adequate to read correspondence, computer screen, forms, etc.
    • Specific vision abilities required by this position include the ability to adjust focus
    • Occasionally required to stand and climb or balance
    • Occasionally lift and/or move up to 25 lbs
