Ventura County Adult Day Health Care is a community-based program providing a variety of medical, therapeutic, rehabilitative and recreational programs and services for adults 18 and older. We provide a safe and secure environment for adults to spend time under the cared supervision of experienced staff. Our mission is to help prevent premature institutionalization by offering stimulating and enjoyable group experiences for participants to maintain their independence and also to provide respite for family members and caregivers. Duties and Responsibilities:

Assess the participants to determine the needs, wishes, and desires for appropriate care for the participants attending to ADHC/CBAS center by developing and implementing Individual Care Plan

Working as a member of the interdisciplinary team and focus on person-centered care practice.

Conducting the wellness check and risk assessment to the participants might provide in-home and in-center services base on the needs of the participants.

Act as a liaison with participants Primary Care Providers.

Report to MDT and Program Director on a regular basis or as needed.

Provide professional in-service training to the staff members and education to the participants.

Qualifications:

RN or LVN licensed by the State of California

Prefer 2 years+ CBAS and ADHC experience

Bilingual: English/Spanish or Tagalog preferred

Be a team member and have the understanding and passion of older adults and families.

The required license and/or certification and references per request.

Salary: $32.00 - $44.00 per hour
