General : Remarkable Hospice provides care to terminally ill patients in their home. Settings include private homes, nursing facilities, assisted living, memory care, senior apartments and other settings.
Duties include:
- Assessing and providing hands on care
- Admissions after hours
- After hours discharges including death pronouncement
- Communicating with the interdisciplinary team
- Documenting necessary information in the electronic healthcare record
- Collaborating with the hospice physician
- Coordinating necessary ancillary services such as DME, lab, xray, transportation
- Providing services collaboratively with other healthcare providers such as in nursing facilities, hospitals and home health
- Oversight and direction of unlicensed assistive personnel
- Education and support for family, caregivers and facility staff
Requirements :
- Position requires dependable transportation without restriction.
Other requirements include:
- Unencumbered licensure in Texas. License in compact state will be considered.
- Current healthcare provider CPR
- Ability to perform essential functions of job with or without reasonable accommodation
- Must pass criminal history check and other required background checks
- Drug and alcohol screening
- One year of registered nursing experience. Two years with hospice preferred
- Bilingual English and Spanish or other languages a plus
- Homecare Homebase experience a plus
