As part of our commitment to innovation and excellence, we are expanding our Chronic Care Management (CCM) program and are seeking a Remote Medical Assistant to support our care management team.

Key Responsibilities:
- Assist patients by providing information, coordinating services, and offering support remotely.
- Conduct outreach to patients for follow-ups, wellness checks, and care coordination.
- Maintain and update medical records, ensuring accuracy in electronic medical records (EMR) systems.
- Support physicians and care managers by relaying patient updates, scheduling appointments, and ensuring smooth care transitions.
- Educate patients on managing chronic conditions and adhering to treatment plans.
- Maintain patient confidentiality and comply with HIPAA and healthcare regulations.
- Monitor and manage inventory of medical supplies for remote support, if needed.
- Stay updated with industry best practices in telehealth and chronic care management.
Qualifications:
- Certified Medical Assistant (CMA) or equivalent.
- 2-3 years of experience in a medical office, preferably in chronic care management or telehealth.
- Proficiency in EMR systems and medical terminology.
- Strong verbal and written communication skills.
- Bilingual in Spanish highly preferred.
- Experience working in a remote healthcare setting is a plus.
Compensation & Benefits:
