Overview: We are seeking a skilled and dedicated Nurse Practitioner to join our healthcare team. As a Nurse Practitioner, you will play a vital role in providing high-quality patient care and promoting wellness. This position is based in a office setting and requires expertise in Remote Patient Monitoring

Responsibilities:

• Conduct comprehensive medical assessments

-manage a primary care facility for Medicare and Medicaid patients

• Diagnose and treat acute and chronic illnesses
• Develop and implement patient care plans
• Provide health coaching and education to patients and their families
• Collaborate with healthcare professionals to coordinate patient care
• Utilize electronic medical records system for documentation and coding

Requirements:

• Valid Nurse Practitioner license in [state]
• Master's degree in Nursing
• Minimum of 2 years of experience as a Nurse Practitioner

-Have its own NPI for billing purposes

• Excellent communication and interpersonal skills
• Ability to work collaboratively within a multidisciplinary healthcare team
