Position Summary:
Balance Assisted Living in Lodi, CA, is seeking a Resident Care Coordinator to oversee the delivery of exceptional care to residents while coordinating staff and supporting high standards of care. The Resident Care Coordinator will be responsible for developing and implementing care plans, managing caregiver and med tech schedules, and ensuring all residents receive personalized care in line with our person-centered philosophy. Responsibilities of the Resident Care Coordinator:
- Leadership and Communication: Provide leadership, exhibiting professional communication with caregivers, residents, families, and healthcare professionals.
- Care Coordination: Develop and update resident care plans, ensuring care is tailored to individual needs and preferences.
- Activities of Daily Living (ADL): Oversee and provide assistance with daily activities such as bathing, grooming, and dressing while promoting independence.
- Documentation and Reporting: Accurately document care provided and report any changes in resident conditions or needs.
- Team Development: Recruit and train new Caregivers and Med Techs; conduct ongoing performance evaluations and provide regular feedback.
- Schedule Management: Ensure staffing schedules are completed, maintaining appropriate caregiver and med tech coverage.
- Resident Interaction: Address resident, family, and responsible party concerns in a timely and professional manner.
- Training and In-services: Participate in employee orientation and regular in-service training to maintain a high level of care and staff competency.
- Compliance: Ensure compliance with all safety, infection control, and regulatory procedures.
- Support: Fill in for shifts as needed to ensure the well-being of residents.
Required Experience:
- Education: High school diploma or equivalent (required).
- Experience: Minimum of two (2) years of senior living experience preferred. At least one (1) year of experience as a Caregiver or Med Tech preferred.
- Age Requirement: Must be at least 18 years of age.
- Skills: Basic computer skills; must be able to read, speak, and write in English.
- Background Check: State Criminal Background Check and LIC 508 Criminal Record Statement required.
- Health Screen: LIC 503 Health Screening Report required.
