    The Resident Coordinator Activities and Appointments plays a vital role in ensuring the well-being and engagement of residents across Jmkay Healthcare’s facilities. This position is responsible for planning and organizing meaningful activities that promote social interaction, wellness, and independence, as well as coordinating and scheduling medical and specialty appointments to ensure residents receive the necessary care and support. A typical day in this role may involve creating and updating the monthly activity calendar, arranging transportation for resident outings, or leading group recreational activities. Additionally, the coordinator will manage medical appointments, ensuring proper scheduling, documentation, and communication between staff, healthcare providers, and families. The role requires collaboration with Direct Support Professionals (DSPs), Facility Program Managers, and Designated Coordinators to align activities and appointments with individual care plans and organizational policies.

    This is a dynamic, hands-on position that demands excellent organizational skills, communication abilities, and a passion for improving residents' quality of life. The ideal candidate should have at least one years of experience in a healthcare, social services, or assisted living environment, particularly in activity planning or appointment coordination.     Job Responsibilities

    Plan and Organize Resident Activities – Develop and manage monthly activity calendars, special events, and community outings that align with residents' interests and CSSP goals.

    Coordinate and Schedule Medical & Specialty Appointments – Manage and document all medical, dental, and therapy appointments while coordinating transportation and follow-ups.

    Collaborate with Facility Staff for Seamless Operations – Work with Program Managers, Designated Coordinators, and DSPs to integrate schedules and ensure smooth facility operations.

    Monitor Resident Engagement & Well-being – Track participation, assess engagement levels, and implement strategies to enhance socialization and prevent isolation.

    Maintain Accurate Records & Compliance – Keep detailed records of activities, appointments, and resident progress while ensuring compliance with MN 245D and HIPAA regulations.

    Coordinate Transportation & Logistics – Arrange and oversee transportation for medical visits and outings, maintaining relationships with service providers.

    Support and Encourage a Positive Resident Experience – Advocate for residents' needs, gather feedback, and enhance program offerings to improve their quality of life.

    Assist in Budgeting & Resource Allocation – Track expenses, manage budgets, and seek community partnerships to expand engagement opportunities.

    Job Requirements

    Education & Experience – High school diploma required; Associate’s or Bachelor’s in healthcare or social services preferred, with 1-2 years of related experience.

    Organizational & Time Management – Ability to plan, prioritize, and coordinate multiple tasks efficiently across several facilities.

    Communication & Interpersonal Skills – Strong verbal and written communication to interact professionally with residents, staff, and healthcare providers.

    Problem-Solving & Adaptability – Ability to handle scheduling conflicts, unexpected challenges, and changing resident needs effectively.

    Technical & Documentation Skills – Proficiency in Microsoft Office, scheduling software, Rtask and maintaining compliance with HIPAA and MN 245D regulations.

    Collaboration & Leadership – Experience in team coordination, training staff, and working closely with facility managers and DSPs.

    Valid Driver’s License (Required) – Coordinating transportation for medical appointments and off-site activities.

    Resident-Centered Approach – Passion for improving residents’ well-being through meaningful engagement and proactive healthcare coordination.
