    RESIDENT SUPPORT MANAGER

    We are seeking a compassionate and dedicated Resident Support Manager to join our team.

    EXPERIENCE

    knowledge of state of Tennessee DDA rules and regulations; a minimum of 3 years’ experience working as a home manager or lead, or in similar residential programs; excellent written, verbal and organizational skills.

    EDUCATION

    Associates degree in a psychology, human services, education or related and 1 year of experience working with people with intellectual and developmental disabilities or three years of experience working as a Home Manager or lead; work experience may be substituted for education.

    TRAINING AND PERSONNEL REQUIREMENTS

    It is the responsibility of the RSM to ensure all of Thrive Homes, Inc., DDA and state licensure requirements are met and maintained on a timely basis.

    RELIAS Training courses

    All personnel documents (valid drivers license with continued good driving record, clear annual background checks, TB skin test questionnaire, adequate coverage of automobile liability insurance, etc. as requested by HR)

    RESPONSIBILITIES

    Abides by and demonstrates the company’s Mission, Vision, and Values through both behavior and job.

    Monitor the documentation of daily shift notes from direct care staff for not only presence, but for quality to reflect required content for services provided (meet billing standards and definition of expected services provided ) and to ensure person’s outcomes-goals are being implemented as outlined in the PCSP.

    Write and submit Provider Monthly Reviews timely as required by DDA.

    Conduct and document on required form, weekly and monthly supervisory home visits in accordance with frequency of service required by DDA and TennCare standards.

    Ensure outstanding issues from any home supervisory visits are resolved timely and are documented until resolution. (maintenance, staff conduct, Medication orders and MARS, any missing required documents, cleanliness, person centered care issues, etc.).

    Ensure the health and safety of persons supported via medical oversight of Direct Support Staff (DSP’s), Family Model Providers (FMPS’s), House Managers (HM’s); including ensuring the tracking of timely medical assessments, annual and routine, and any specialty care as needed.

    Ensure Medication Administration Records are in accordance with doctors orders and completed accurately for all persons supported that Thrive staff is responsible for administering medication.

    Ensure that DSP and FMPS are conducting all environmental safety requirements such as Fire Drills monthly, and all homes have fire extinguishers, first aid kits, safe vehicles, smoke detectors, carbon monoxide detectors, 3 or more days of food, etc.

    Ensure all weekly and monthly paperwork is completed and uploaded to the format provided by Thrive Homes, Inc. such as, but not limited to, person supported record requirements, monthly provider reviews, home visits, medical assessments, MARS, physician orders, vehicles checks, fire drills, temperature checks, consent forms, leases, all new admission documents and timeliness of all annual required documentation.

    Follow Event Reporting protocol and attend REF committee meetings; complete REFs and submit timely when requested by Thrive Administration. Cooperate with Agency and DDA Staff during any inspection or investigation.

    Act as liaison on behalf of Thrive and each person we serve by communicating with Care Coordinators/ISC’s, therapists, conservators, family members, medical personnel and all circle of support members, on their caseload, in a positive and respectful manner; maintain communication as needed to provide quality services to each person.

    Advocate and coordinate as necessary for people supported involvement in support groups, self-advocacy, employment services, transportation services, community activities, recreation and learning independent living skills.

    Advocate and promote community integration through encouraging persons supported to be active in their community, to explore their community and to assist DSP's in offering various opportunities and activities that build on the persons interests, preferences, gifts, and strengths, while reflecting the persons goals in the community involvement, membership in exploring employment interest.

    Attend manager, supervisory and staff meetings; ensure dissemination of all information to staff and others.

    Initiate and attend Meet and Greets for referrals and follow up with supervision and Thrive Administration for meeting outcomes.

    Coordinate/attend as needed all person related meetings (IHP, IDT, case manager, circle, house, family, etc.); ensure all necessary follow-up is completed.

    Coordinate all aspects of program financial procedures including monitoring of utility use in accordance with agency and DDA policies.

    Coordinate all aspects of financial procedures for the people we serve in accordance with agency and DDA policies. Assure the proper and prudent expenditure of all funds.

    Must adequately function independently with minimum supervision, and execute effective time management to ensure all duties are completed timely.

    Perform additional duties as required by the nature of the position or as requested by supervisor

    SUPERVISION/TRAINING RESPONSIBILITIES

    Monitor staff performance management, including evaluations and developmental plans, disciplinary actions, etc., for all program staff as scheduled.

    Conduct admissions process for all new persons supported, their staff and families, on the assigned case load, by ensuring staff (DSPs and FMPs) have completed all new admissions documentation, and staff and persons supported are trained to persons rights, persons specific needs, medical-diagnosis education and all policies and procedures as applicable.

    Train DSPs and FMPs to all required paperwork, goal tracking and daily note requirements, as well as REF reporting and reporting requirements of all applicable events.

    Supervise staff training and development through teaching, acting as a role model and guiding staff on how to appropriately work with and interact with persons we support, other staff, families and guardians.

    Present and implement all agency policies and procedures, and DDA requirements.

    Inform supervisor as needed about pertinent issues.

    Coordinate staff and replacement staff schedules.

    Carry on call phone on rotational schedule; respond while on-call to program needs including covering open shifts.
