    We are seeking a complete Rehabilitation Therapy Team (PT, PTA, OTR, COTA, SLP, LCSW, ASW) in SAN DIEGO COUNTY, CA to become a part of our team! You will provide routine healthcare to patients at the patient's home or in a care facility.

    Busy Home Health Agency in need of PT, PTA, OTR, COTA, SLP, LCSW, ASW that reside in SAN DIEGO COUNTY, CA. Requires 1 year of home health, hospital, nursing home and/or hospice experience. Flexible schedule and possible full-time/part-time/per diem positions available.

    --- Job Title/Position: Physical Therapist ---

    JOB DESCRIPTION SUMMARY: The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

    POSITION QUALIFICATIONS: 1. Possesses a degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization. 2. Licensed to practice as physical therapist by the Physical Therapy Board of California. 3. Two years of appropriate experience as a physical therapist. Community/home health experience is preferred. 4. Demonstrates good verbal and written communication and organization skills. 5. Possesses and maintains current CPR certification. 6. Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

    --- Job Title/Position: Physical Therapy Assistant ---

    JOB DESCRIPTION SUMMARY: The physical therapy assistant provides physical therapy services under the direction of the physical therapist and according to the physician's plan of care.

    ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:

    1. Provides direct physical therapy according to directions of a qualified physical therapist and in accordance with the physician's plan of care and as defined in the state Physical Therapy Practice Act.

    2. Treats patient to relieve pain, develop or restore function, and maintain maximum performance.

    3. Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training.

    4. Utilizes physical agents, including, but not be limited to heat, cold, water, light, electricity, ultrasound and massage.

    5. Observes and reports to the physical therapist and the Organization personnel the patient’s reaction to treatment and any changes in patient’s condition.

    6. Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.

    7. Maintains necessary records; clinical notes and conference notes, which will be incorporated into the patient’s clinical record in prescribed time frames.

    8. Participates in Organization Home Care Team Members meetings and Organization Inservice Education Program.

    9. Performs other duties as assigned.

    POSITION QUALIFICATIONS: 1. Graduate of an accredited physical therapy assistant program with either an Associate’s degree in science or applied science that is approved by American Physical Therapy Association, Inc. 2. Approved as a physical therapist assistant by the Physical Therapy Board of California. 3. Demonstrates good verbal and written communication and organization skills. 4. Possess and maintain current CPR certification. 5. Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

    --- Job Title/Position: Speech–Language Pathologist ---

    JOB DESCRIPTION SUMMARY: The speech–language pathologist is responsible for implementation of standards of care for speech language pathology services and for adherence to all conditions in the Service Agreement.

    POSITION QUALIFICATIONS: 1. Possesses a master’s or doctoral degree in speech-language pathology (SLP), and who meets either of the following requirements: A. Is licensed as a speech-language pathologist by the State in which the individual furnishes such service; or B. If the State does not license speech-language pathologists: 1. Must successfully complete 350 clock hours of supervised clinical practicum (or is in the process of accumulating such supervised clinical experience); 2. Performed not less than 9 months of supervised full-time SLP services after obtaining a master’s or doctoral degree in SLP or a related field; and 3. Successfully completed an approved national examination in SLP 2. A minimum of one (1) year's clinical experience in Speech–Language Pathology. 3. Demonstrates good verbal and written communication, and organization skills. 4. Possesses and maintains current CPR certification. 5. Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

    --- Job Title/Position: Occupational Therapist ---

    JOB DESCRIPTION SUMMARY: The occupational therapist contracted through the Organization is responsible for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the Service Agreement. Any direct patient care provided will be performed as defined by the state Occupational Therapy Practice Act.

    POSITION QUALIFICATIONS: 1. Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association. 2. Registered with the National Registration Examination of the American Occupational Therapy Association. 3. Licensed as an occupational therapist by the California Board of Occupational Therapy. 4. Two years of clinical experience as an occupational therapist. 5. Demonstrates good verbal and written communication, and organization skills. 6. Possess and maintains current CPR Certification. 7. Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

    --- Job Title/Position: Certified Occupational Therapy Assistant ---

    JOB DESCRIPTION SUMMARY: The certified occupational therapy assistant contracted through the Organization is responsible to the registered occupational therapist that is responsible for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the Service Agreement. The certified occupational therapy assistant performs only those services planned, delegated, and supervised by the registered occupational therapist.

    POSITION QUALIFICATIONS: 1. Graduate of an occupational therapy assistant curriculum accredited by the American Occupational Therapy Association. 2. Certified with the National Certification Examination of the American Occupational Therapy Association. 3. Licensed as an occupational therapy assistant by the California Board of Occupational Therapy. 4. A minimum of one (1) year of clinical experience as a certified occupational therapy assistant. 5. Demonstrates good verbal and written communication, and organization skills. 6. Possesses and maintains current CPR certification. 7. Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

    --- Job Title/Position: Social Services Supervisor/Medical Social Worker/Licensed Clinical Social Worker ---

    JOB DESCRIPTION SUMMARY: Responsible for managing all aspects of the provision of patient social services including managing the members of the social work team. Establishes, implements, and evaluates goals and objectives for social services to meet and promote the standards of quality and contribute to the organizations mission and philosophy.

    POSITION QUALIFICATIONS:

    1. Licensed mental health professionals include Marriage and Family Therapists (MFT), LCSWs, Licensed Clinical psychologists, and licensed physician certified in psychiatry by the American Board of Psychiatry and Neurology. Clinical Social Worker with the CA Board of Behavioral Science as specified in CA B&P Code Section 4999.12) 2. Effective April 1, 2013, a person supervising an associate clinical social worker (ASW) must maintain current valid California license and be licensed for a total of at least two years prior to commencing any supervision and have practiced clinically in two of the past five years immediately preceding the commencement of supervision.

    JOB DESCRIPTION SUMMARY: The Social Worker/Social Work Associate/Associate Social Worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement.

    POSITION QUALIFICATIONS: 1. Graduate of a Master’s or doctoral program in social work accredited by the Council on Social Work Education. Meets California requirements Section 1870 of California Code of Regulations. 2. Meets personnel qualifications stated in §418.114(b)(3). 3. Minimum of one year’s experience in health care, home care experience preferred. 4. Experience in a home care preferred. 5. Demonstrates good verbal and written communication, and organization skills. 6. Possesses and maintains current CPR Certification. 7. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
