    Assess and evaluate the needs of clients.

    Formulate individual service plans for caseload in collaboration with team, parents, client, and placing agencies.

    Coordinate and implement service plans.

    Conduct individual, group, and family therapy according to each client’s treatment plan.

    Attend IEP meetings when appropriate.

    Continue appropriate services when the client is psychiatrically hospitalized.

    Consult and collaborate with appropriate staff and consultants.

    Ensure that all caseload files are up to date with all required documentation.

    Be consistent and on time with the documentation/writing of all reporting documents.

    Maintain consistent and effective communication with families on caseload, placing agencies, and other agency personnel.

    Make appropriate admission and termination recommendations.

    Maintain a positive relationship with Agency staff, as well as personnel with the San Bernardino Department of Behavioral Health.

    Maintain a positive relationship with treatment team members, as well as other agency personnel.

    Utilize all appropriate services for clients, as indicated.

    Participate in all required in-service training and assist with the facilitation of same when requested.

    When assigned, provide coverage for clients on other caseloads.

    Maintain a Single Fixed Point of Responsibility for the Agency.

    Job Description

    Participate in Utilization Review and Quality Assurance processes, as indicated.

    Participate in weekly clinical case staffing, case conferences, and consultation meetings.

    Contribute to the education and training of direct care workers and other staff professionals through workshops, lectures, and other educational activities, as deemed by the Head of Service.

    Skilled in handling emergencies, including recognition of the severity of a crisis.

    Other duties and responsibilities as assigned.

    Maintain and update all para-professional release of information documents for the clients to not break Confidentiality/HIPPA requirements.
