    Scope:

    We are seeking a dedicated and skilled Physical Therapist (PT) to join our dynamic healthcare team! The successful candidate will be responsible for assessing, planning, and implementing rehabilitation care plans to help patients recover from injuries, illnesses, and surgeries. The PT will work closely with a team of healthcare professionals to ensure a comprehensive approach to patient care. Able to work autonomously as well as within an interdisciplinary team to apply a variety of techniques and methods in the treatment of the patient. The PT will provide care consisting of appropriate exercises, procedures, and techniques within the facility to promote effective recovery and optimize patient outcomes. The PT encourages and inspires their patients to work toward greater independence and recovery. The PT must adhere to established policies, procedures, standards of practice, and will provide care within the scope of practice.

    All healthcare services shall be provided in accordance with established standards, principles, regulations, best practices, and ethics of the profession.

    Responsibilities:

    · Perform initial and ongoing physical therapy evaluations according to American Physical Therapy Association (APTA) standards of practice as well as the Missouri Department of Health standards.

    · Develop personalized treatment plans to help patients restore function, alleviate pain, and improve mobility.

    · Conduct a comprehensive assessment to identify impairments, functional limitations, and contributing factors to develop a holistic understanding of patient's condition.

    · Utilize a variety of interventions, including therapeutic exercises, manual therapy techniques, modalities, and patient education, to address impairments, improve function, and promote overall well-being.

    · Perform hands-on manual therapy techniques, such as joint mobilization, soft tissue mobilization, and manual stretching, to address musculoskeletal dysfunctions and improve tissue mobility.

    · Promote therapeutic exercises, functional activities, and neuromuscular re-education exercises to enhance strength, flexibility, and motor control.

    · Develop individualized care plans based on assessment findings for treatment modalities to patient goals, closely monitoring their responses and adjusting treatment parameters as necessary using evidence-based practice guidelines.

    · Guide patients through functional mobility training, gait training, and Activities of Daily Living (ADL) retraining to improve their ability to perform essential tasks and participate in meaningful activities.

    · Incorporate real-life scenarios and environmental modifications to facilitate the transfer of skills to home and community settings.

    · Implement plan of treatment on a consistent schedule and adjust this schedule and plan according to patient progress and requirements.

    · Evaluate mobility, strength, and current level of patient function and develop goals which are realistic, attainable, and are scaled to patient’s functional abilities.

    · Monitor patient’s condition and document observations accurately.

    · Collaborate with healthcare team members to ensure patient needs are met.

    · Use a variety of techniques, including exercises, manual therapy, and modalities, to improve patients' mobility, strength, and overall physical function.

    · Educate patients and their families on treatment plans, home exercises, and lifestyle modifications to support recovery and prevent future injuries.

    · Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.

    · Maintain accurate and up-to-date patient records, including treatment plans, progress notes, and discharge summaries.

    · Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and comprehensive care.

    · Stay current with advancements in physical therapy techniques and practices through continuing education and professional development.

    · Coordinate patient care and prioritize tasks as needed.

    · Maintain all medical records in accordance with privacy laws, healthcare protocols and policies.

    · Ensure patient safety through adherence to infection control practices and other safety protocols.

    · Communicate effectively with patients, families, and interdisciplinary team members.

    · Ensure discharge plan is individualized and appropriate for the population served.

    · Comply with all applicable Summit Healthcare Solutions standards and guidelines.

    · Other duties as assigned within scope of practice.

    Qualifications:

    · Bachelor’s degree from an accredited Physical Therapy program, approved by the American Physical Therapy Association.

    o Preferred: Master’s degree from an accredited Physical Therapy program, approved by the American Physical Therapy Association.

    · Current unrestricted Physical Therapy license for the State of Missouri.

    · Two (2) years of Physical Therapy experience.

    · Current Basic Life Support (BLS) certification.

    · Ability to understand instructions furnished in written and oral forms of English.

    · Proficiency in the use of basic Microsoft word, Outlook, and Excel.

    · Ability to pass a background check and drug screening.

    Physical Requirements:

    · Physical fortitude to lift patients with assistance using proper body mechanics.

    · Routinely performed in an interior clinic / office environment.

    · May involve sitting for prolonged periods of time.

    · May require standing and walking for prolonged periods of time.

    · May require bending, stooping and lifting up to 25 pounds.
