Your Role as an RN:

• Develop and implement personalized patient care plans.
• Conduct comprehensive assessments and support families with education and resources.
• Coordinate home health services to ensure seamless, high-quality care.
• Collaborate with a dedicated healthcare team to achieve the best patient outcomes.

Requirements:

• Active Arizona RN License
• Valid Driver’s License & Reliable Transportation
• Current CPR/BLS Certification (or willingness to obtain)
• TB & Fingerprint Clearance (assistance provided)
• Home Health Experience Required
• Bilingual (Spanish) is a plus but not required
• Graduate of an accredited RN program
