Role Description
This is a part-time, on-site role for a Care Manager located in Bedford, TX. The Care Manager will be responsible for overseeing and coordinating care for clients, particularly the elderly. Day-to-day tasks will include case management, facilitation of social services, training new caregivers, and ensuring the overall well-being of clients. The role will also involve regular interactions with clients, families, and other healthcare professionals to create and maintain comprehensive care plans.
Qualifications
1 year of Case Management and Social Services skills
2 years of Elder Care expertise and a background in Medicine or In-Home Care
Experience in Training caregivers
Strong communication and interpersonal skills
Ability to work effectively as part of a team
Licensed Vocational Nurse
Bachelor's degree in Social Work, Nursing, or a related field
Experience in home care or healthcare settings is a plus
Reliable Transportation
Skills
Strong communication, interpersonal, and organizational skills.
Ability to work independently and as part of a team.
Proficiency in electronic health record (EHR) systems.
Knowledge of relevant healthcare regulations and standards.
Empathy, compassion, and a patient-centered approach.
