Senior Director Care Management

Denver, Colorado

The Senior Director of Care Management directs the strategic, growth, and operational objectives for the Care Management service line in conjunction with organizational strategic plans. The Senior Director of Care Management directs and oversees continuous improvement initiatives, utilizing an inter-disciplinary and multi-departmental approach, to achieve quality and superior patient care and services across the Continuum of Care.

Qualifications:

• Registered Nurse preferred
• Master's degree in Nursing, Healthcare or Master's degree in Social Work, Required

• Senior/Executive Director of Care Management experience in a Multi-Site Director role
• Director of Case Management experience in an acute hospital setting
  

Responsibilities:

• Develops and directs strategic growth and operational objectives for the service line
• Oversees department operations; integrates in/outpatient services
• Directs and oversees program development, performance management, personnel activities, budget, and the provision of services to continually achieve higher levels of excellence and cost effectiveness
• Develops and empowers management teams to lead daily operations, resolve issues, manage employee performance and physician relations
• Promotes quality patient care outcomes while supporting appropriate utilization/resources management
