  • Responsibilities

    Duties: The Service Coordinator:

    Assists with the development and implementation of Person-Centered Individualized Support Plans (PCISP).

    Assists with the design and implementation for the individual’s support and assisting with teaching strategies targeted at assisting individuals with reaching their outcomes as outlined in the individual’s PCISP;

    Supervises and trains DSPs on the implementation of the PCISP, ISP outcomes, and teaching strategies;

    Assists with the revision of the PCISP and ISP outcomes as necessary based on data collection, feedback from staff, and direct observation of the individual. Outcomes may only be changed with approval from the individual, the individual’s guardian, and members of the interdisciplinary team.

    Attending the Individualized Support Team (IST) meetings.

    Ensures that services and support are provided as stated in the PCISP.

    Prepares and submits reports that are required by contract, funding sources, or regulations including but not limited to preparing and uploading service summaries to the Bureau of Disabilities Services (BDS) portal.

    Maintains individual file at home and the office.

    Reports Incident Reports to BDS as applicable and provides oversight for follow up plan with staff.

    Coordinates in-service training needed to assist staff in fulfilling responsibilities related to individual directed support.

    Ensures monthly staff schedule is completed with appropriate level of staff coverage.

    Provides direct support care as needed to protect the health and safety of the individuals served in a staffing crisis.

    Provides oversight for Team Leads and DSPs.

    Completes performance evaluations for HSA, TLs, and DSPs. Submits annual evaluations to Human Resources one month prior to hire date anniversary to ensure timely reviews.

    Communicates with the Individualized Support Team about concerns and findings;

    Is available to respond to staff in case of an emergency.

    Submits Oversees coordination of individual benefits including Medicaid, Medicare, SSI, SSA, and Food Stamps.

    Secures community resources for individuals supported.

    Visits supported living sites at least monthly to perform health and safety inspections.

    Qualifications: Must have a high school diploma, associate's degree preferred. One (1) year experience in the field of intellectual and developmental disabilities required one (1) year of supervisory experience preferred.
