    Job Description

    Comfort Home Care Solutions, a healthcare agency in San Antonio, TX, is seeking a dedicated and detail-oriented individual for the role of Service Supervisor and Staffing Coordinator. This dual role is integral to our team, driving quality in client care and services while promoting the company's growth.

    Job Duties and Responsibilities

    Coordinate and schedule the workload of staff members and contractors, maximizing personnel capacity.

    Provide daily supervision of caregiver employees to maintain high-quality services and adherence to standards.

    Review and investigate client records (clinical content, diagnoses, medical history, medications, fees) for accuracy and compliance.

    Direct staff to appropriate community resources as necessary.

    Conduct performance appraisals and provide teaching and counseling to enhance service delivery and staff development.

    Evaluate client needs and assess the suitability of home care based on intake screening forms.

    Develop and review comprehensive care/service plans with clients or their family members.

    Participate in community-related functions and orient new professional staff.

    Secure physician orders for continued care and provide client visits as needed.

    Ensure all necessary paperwork is stored in client records.

    Contribute to the Agency's continuing education program through in-service education and training.

    Coordinate client visits and maintain scheduling records and logbooks.

    Travel to potential clients' homes to conduct intake assessments.

    Communicate scheduling changes accurately and promptly between field staff and the office.

    Prepare weekly schedules for field staff and distribute them.

    Monitor schedule changes and service delivery to clients.

    Ensure completed visits comply with policies and report results to the supervisor.

    Assist in referral processing and provide necessary paperwork to the Payroll/Billing Manager and on-call coordinator.

    Assist with marketing support by identifying and pursuing new client opportunities through outreach, events, and in-person opportunities.

    Job Conditions

    Primarily desk work with occasional lifting of medical supplies and client documents.

    Travel is necessary for home visits, scheduled supervision of care/services, and other business-related tasks.

    Qualifications

    Training/experience in providing Personal Care Services.

    Experience with provider portals for EVV (e.g., HHAExchange, Vesta, etc.) is required.

    A minimum of two (2) years of supervisory or administrative experience in home care, private duty, environmental support, or related health programs.

    Experience with processing medicaid and private pay clients is required.

    Ability to meet initial and ongoing educational requirements as mandated by the state.

    Ability to work under stress to meet deadlines.

    Effective verbal and written communication skills.

    Creative problem-solving skills and attention to detail.

    Ability to work independently and as part of a team.

    Strong organizational skills and supervisory experience.
