We are seeking a dedicated and compassionate Social Worker to provide coverage across multiple healthcare facilities as needed. The Social Worker will perform comprehensive psycho-social assessments, develop and implement care plans, and provide intervention, management, and support for at-risk patients and their families. This role involves working closely with the multidisciplinary team to address social determinants of health, discharge planning, and aftercare needs.

Key Responsibilities

• Assess patient needs and develop care plans.
• Coordinate discharge planning and aftercare.
• Address social determinants of health.
• Provide emotional support and advocacy.
• Maintain documentation and perform other duties as needed

Qualifications

*  **Education:** Master's degree in Social Work (MSW) from an accredited institution.
*  **Licensure:** LCSW or LMSW preferred, based on state requirements.
*  **Experience:** 3 years in a hospital and/or healthcare setting.
