Job Description Position Title: Social Worker
Position Summary
Under general direction, the Social Worker manages complex caseloads requiring advanced social services skills. This includes determining the need for social services, conducting in-depth assessments, developing and implementing care plans, and connecting clients with necessary resources. The Social Worker III handles challenging cases involving individuals and families with multifaceted needs and acts as a lead worker when required. This position demands high levels of technical competence, independent judgment, and familiarity with community resources.
Key Responsibilities
Case Management • Carry a caseload of complex social services cases, including those affected by challenging environmental factors. • Develop, implement, and monitor social treatment plans tailored to individual and family needs. • Investigate reports of abuse, assess safety risks, and recommend alternate placements as necessary. • Provide crisis intervention, support, and guidance to individuals and families.
Collaboration and Advocacy • Collaborate with community organizations, law enforcement, and other stakeholders to address client needs. • Act as a liaison for clients with external resources, including legal, medical, and social service entities. • Advocate for client access to appropriate services while addressing barriers to care.
Documentation and Reporting • Prepare and maintain comprehensive case files, including assessments, reports, and safety plans. • Submit court reports and testify as needed in legal proceedings. • Enter and retrieve data from electronic systems to maintain accurate client records.
Supervisory and Training Responsibilities • Provide guidance and act as a lead worker for less experienced social service workers. • Participate in staff development activities and training to enhance technical competence.
Additional Duties • Conduct home visits to monitor compliance with care plans. • Develop and maintain client confidentiality per the National Association of Social Workers (NASW) Code of Ethics. • Perform related duties as assigned.
Required Skills and Qualifications Knowledge of: • Socio-economic conditions and trends affecting client behavior and well-being. • Public welfare programs and policies at federal, state, and local levels. • Techniques of interviewing, case management, and social work practices. • Laws and regulations governing public social services. • Crisis intervention strategies and mental health services.
Skills: • Effective communication, both written and verbal, including report writing. • Analytical and problem-solving abilities to assess client situations and develop appropriate plans. • Proficiency in electronic health records and case management software. • Ability to work under stressful conditions and handle sensitive cases.
Minimum Qualifications Education and Experience: • One year of full-time social work case management experience at the journey level. • Or Two years of full-time case management experience in a public or private agency and completion of 30 semester units in a relevant field. • Operate standard office equipment and personal computers. • Frequently sit for extended periods and occasionally lift or carry up to 40 pounds. • Drive and make home visits as part of casework responsibilities.
