    Licensed Social Worker LCSW Full Time Days - Joshua Tree, California

    Full Time

    Shift: 8:00am - 4:30pm

    Schedule: Weekdays and every other weekend. Some holidays required.

    Position Summary

    The individual in this position is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and interventions to promote timely throughput, safe discharge and prevent avoidable readmissions. This position: integrates national standards for case management scope of services including:

    • Transition Management promoting appropriate length of stay, readmission prevention and patient satisfaction
    • Care Coordination by demonstrating throughput efficiency while as: wring care is sequenced and provided at the appropriate level of care
    • Compliance with state and federal regulatory requirements, TIC accreditation standards and Tenet policy
    • Education provided to physicians, patients, families and caregivers
    • Precepts new staff members and acts as resource to all staff.
    • Participates in department Quality Improvement initiatives, one committee participation and/or major projects as assigned

    Responsibilities

    • LCSW license required.
    • Accredited Case Manager (ACM) preferred.
    • Minimum of two years acute hospital experience preferred.
    • Required skills include demonstrated organizational skills, excellent verbal and written communication skills, ability to lead and coordinate activities of diverse group of people in a fast paced environment, critical thinking and problem solving skills and computer literacy.

    Social Worker LCSW Full Time Nights

    Palm Springs, California

    Under general direction of the Social Work Manager / LCSW, delivers age-appropriate social work care to patients and their caregivers, in accordance with hospital policies and procedures, and state and federal regulations.

    The Social Worker II serves as an integral member of the healthcare team providing assessments, coordination, treatment planning, information and referral to community resources, and other social work services to meet the complex needs of patients and families in the hospital and clinic settings.

    MSW will seek out supervision as needed with a Licensed Clinical Social Worker.

    Will use Patient Medical Record and HPF as primary information systems.

    The individual in this position performs psychosocial assessments and develops a plan to meet the needs of patients with complex social issue/problems, and arranges post-discharge services for patients as needed.

    This individual’s responsibility will include, but not limited to the following activities:

    a) assist with adoptions, abuse and neglect cases, including making referrals to state and federal agencies

    b) participate in the weekly Complex Case Review meeting and the daily patient care conferences

    c) provide resources and make referrals to community agencies for ongoing counseling and or continued care

    d) maintain an updated list of providers for post-discharge care

    e) collaborative communication with post-discharge providers

    f) update Case Managers on variance to post-discharge plans

    g) monitor and provide reports to the DCM and hospital leadership as requested

    h) maintain knowledge of regulatory requirements and community resources

    i) and other duties as assigned.
