Job Summary
We're expanding our established primary care services to better address the behavioral and psychosocial needs of our diverse patient population. We're seeking a proactive and entrepreneurial LCSW who is passionate about whole-person care and collaborative practice integration.
This is more than a job — it’s a chance to embed yourself into a high-functioning primary care team, co-develop behavioral health programs, and build a scalable clinical service line. You'll have autonomy to shape how behavioral health fits into the medical home model, with strong support from leadership. You'll collaborate directly with our physicians and chronic care team, manage your own patient panel, and have the freedom to innovate, grow, and potentially establish a long-term partnership.
Responsibilities
Provide brief psychotherapy and evidence-based interventions (e.g., CBT, MI) for individuals with depression, anxiety, adjustment issues, and chronic illness
Implement standards of care for medical social work services
Deliver same-day behavioral health support via warm handoffs
Assist with screening and addressing social determinants of health (e.g., housing, food insecurity, trauma)
Co-manage patients alongside PCPs using collaborative care and BHI models (e.g., 99484, G2214)
Create documentation workflows and billing protocols for psychotherapy and care management codes (90832–90837, 99484, 99490)
Support the clinic’s value-based care performance through integrated behavioral support
Qualifications
Active LCSW license in New York State
Minimum one year of experience in health care and social work
Strong background in primary care behavioral health or medical social work
Working knowledge of behavioral health billing (including incident-to, BHI, CoCM, and psychotherapy CPT codes)
Excellent communicator and collaborator — comfortable working in a fast-paced, multidisciplinary team
Creative, self-directed, and motivated to grow a practice-within-a-practice
Bonus if experienced with EMR-based care plans, patient registries, and quality metrics (e.g., PHQ-9)
