Position Summary

We are seeking a dedicated Social Worker to join our interdisciplinary team. The successful candidate will be responsible for providing comprehensive support to our residents and their families, ensuring high-quality care and services.

Key Responsibilities:

• Coordinate services with various healthcare providers, including dental, optometry, and other necessary referrals.
• Provide professional counseling and emotional support to residents and their families.
• Investigate, intervene, and document resident behaviors in accordance with community protocols.
• Evaluate and address residents' psychosocial needs through thorough assessments.
• Manage discharge planning processes to ensure smooth transitions for residents.
• Assist residents in accessing financial benefits they are entitled to receive.
• Maintain accurate and up-to-date Social Services records for all residents.
• Perform additional duties as assigned by management.

Qualifications

Basic Qualifications & Experience

• Bachelor of Science in Social Work or equivalent work experience.
• 1 - 2 years experience in social work, preferably in a long-term care facility.
• Experience with computer word processing and database programs.
• Must be able to read, write and speak the English language.
• Must possess excellent customer service skills

Working Conditions / Physical Requirements

• Light physical effort required by handling objects up to 20 pounds occasionally and/or up to 10 pounds frequently.
• Sits, stands, bends and moves intermittently during working hours.
• Is subject to frequent interruptions.
• Interacts with residents, family members, visitors frequently.

Social Worker Part-Time $27.05-$32.20
