We are seeking a compassionate and dedicated Social Worker to join our skilled nursing team. The Social Worker will play a crucial role in supporting our residents' emotional, mental, and social well-being. This position involves collaborating with interdisciplinary teams, engaging with residents and their families, and facilitating access to necessary resources and services to enhance the quality of life for those we serve.
Key Responsibilities:
Assessment and Evaluation: Conduct comprehensive assessments of residents' psychosocial needs and establish individual care plans in collaboration with the interdisciplinary team.
Counseling and Support: Provide individual and group counseling to residents and their families, addressing issues such as adjustment to illness, grief and loss, and transition to skilled nursing care.
Resource Coordination: Connect residents and families with community resources, support services, and programs that enhance their quality of life and address specific social, economic, or emotional needs.
Crisis Intervention: Respond to crisis situations as they arise, providing immediate support and intervention to residents and families, and coordinating with appropriate healthcare providers.
Family Support and Education: Offer guidance and education to families regarding residents' conditions, care plans, and available support services.
Documentation: Maintain accurate and up-to-date documentation of assessments, care plans, interventions, and outcomes in compliance with facility and regulatory requirements.
Discharge Planning: Participate actively in discharge planning by coordinating post-discharge services and support for residents transitioning to home or other care settings.
Team Collaboration: Work closely with nursing staff, physicians, therapists, and other healthcare professionals to ensure comprehensive care that addresses the holistic needs of residents.
Advocacy: Advocate for residents’ rights and needs, promoting their dignity and autonomy within the care setting.
Training and Development: Participate in staff training and development programs to enhance social services practices and improve resident care.

Qualifications:
Education: Master’s degree in Social Work (MSW) from an accredited institution.
Experience: Previous experience in a skilled nursing or long-term care setting preferred.
Licensure: Current licensure as a Social Worker (LCSW, LMSW, or equivalent) in the state of Texas.
