POSITION: Substance Abuse Community Case manager
SUPERVISOR: Clinical Supervisor - QMHP, Program Director
SUPERVISEE: N/A
DUTIES: Under the direction of the Program Director, Clinical Supervisor and within the limitations of agency policy, the incumbent shall execute the following duties:
Support clients in all relapse prevention through connection to community resources and supportive services. Assist with care coordination to ensure all vendors are working in collaboration enabling the greatest opportunity for client success.
Mandatory-Contact with each individual client twice (2) a month
Mandatory-Face to Face contact with each individual within at-least every ninety (90) days.
Coordinate and support client to attend, (as needed), medical/psychological appointments
Report changes concerning needs and/or appropriateness for services
Prepare and carry out Individual Service Plans with individuals receiving services.
Document case activity in accordance with program policy and maintain case records in compliance with Agency, regulatory, and accreditation standards.
Assist clients through linking to community resources and developing support networks within the community.
Provide coverage 24 hours per day, 7 days per week with other professional staff.
Participate in supervision with the Program Director and/or /Supervisor as required

$21.00 - $25.00 per hour
QUALIFICATIONS: Incumbent must at minimum be a state registered Qualified Mental Health Professional in Training (QMHP - T) as designated by the Virginia Board of Licensing; have 1 year of experience working with individuals with mental health and/or substance use issues: a valid driver’s license, and possess the following combination of knowledge, skills, and abilities to perform assigned duties:
