Horizons Care Direct Care Staff Supervisor Job Description
Horizons Care provides a comprehensive array of services to include residential foster care, adoption, behavioral health therapy, targeted case management and crisis response to children, families and communities in Manatee, Sarasota, Desoto, Hillsborough and Pinellas counties.
Horizons Care provides effective, flexible and high-quality services in the Out of Home Care program, including 24- hour care in a highly structured residential setting for children and young people with high and complex needs, in accordance with their case and behavioral plan, as well as direct care to foster care clients, foster caregivers, and their families. This role involves 24-hour/7 days a week care and requires staff to be available for a variety of shifts that include overnight stays.
Responsibilities A shift supervisor will oversee the general management of a specific work shift. Supervisor's will be responsible for performance of staff working on his or her shift. They will work directly with lead management to improve shift functions, as well as evaluate direct care staff. They will ensure policy compliance by the staff working under their supervision. They will oversee and properly report client and staff incidents. The supervisor will be responsible for all basic functions including security of the facility , reports, summaries, and logs pertaining to their shift.
The supervisor must adhere to all current federal & state laws, as well as to Horizons Care Policies and Procedures, including the safeguarding of confidential healthcare information and compliance with the Health Insurance Portability Accountability Act (HIPPA).
The supervisor must immediately report any suspected incidents of child abuse or neglect to the Program Administrator and the Child Abuse Hotline.
· Reports serious incidents and updates to caseworkers.
· Attends agency meetings as required.
· Completes other duties as directed by the Program Administrator.
Duties:
Supervise and facilitate all recreational and leisure time.
Provide input in staff meetings to develop individual habilitation plans, as necessary.
Follow individual habilitation and behavioral plans.
Document activities and maintain daily log.
Participate in 40 hours of in-service training annually to include management of aggressive behavior and psychotropic medications as well as 40 hrs. Security training
Immediately report any suspected incident of child abuse, neglect or exploitation to the Child Abuse Hotline and to the Executive Director or Program Director and or staff supervisor.
Assess and correct any problems within skill level.
Ensures the property, grounds, physical workplace areas are maintained to standard.
Performs sudden and unannounced audits on rooms and other operating areas of the facility.
Ensure all safety hazard are properly reported to upper management
