    Job Summary

    The Supportive Services Coordinator is accountable for a variety of responsibilities, including: assisting families engaging in supportive services, helping families to develop the skills, resources and support necessary for long term independence.

    Essential Functions

    • Creates policies and procedures, in conjunction with management for housing case management and supportive services coordination.
    • Conducts housing assessments and needs assessments for program participants.
    • Interprets assessments for the creation of housing plans and service plans for program participants.
    • Monitors program participants to ensure compliance, satisfaction and progress towards agreed upon goals.
    • Protects the rights and confidentiality of program participants.
    • Develops relationships with community agencies, governmental entities, school districts, neighborhood groups and others to connect program participants to resources and opportunities.
    • Assists with marketing Communities First, Inc.’s housing opportunities to the target population and lease up of housing developments.
    • Accurately tracks, collects, and maintains statistics, reports, and activity records. Submits timely reports and maintains records.
    • Meets regularly with program participants to discuss progress, roadblocks and solutions related to agreed upon goals.
    • Assists program participants in interfacing with management to overcome any obstacles that may arise which threaten stable housing, using blended management techniques.
    • Provides or coordinates support services and independent living skills to program participants.
    • Coordinates and schedules monthly calendars for housing developments which meet resident needs (i.e. presentations, workshops, outings, etc.).
    • Participates as an active member of community collaborative groups, tenant councils, etc.
    • Assists in the preparation of applications for funding.
    • Coordinates with senior leadership to define program goals and objectives and evaluate success.
    • Prepares for publication a variety of materials for internal and external use regarding programs, events, activities, etc.
    • Keeps current of changing needs. Remains flexible in ensuring necessary modifications are made.
    • Performs a variety of miscellaneous duties, such as answering phones, typing correspondence, running errands, picking up supplies, assisting senior leadership, and others as needed/directed.
    • Promotes and builds good working relationships with staff of Communities First, Inc. Embraces mission. Works with team; assists whenever and wherever needed/required.
    • Participates in training opportunities to enhance and broaden skillset.

    Education

    Bachelor’s degree in social work, psychology, counseling, human services, or related field

    Required Knowledge, Skills, Abilities, Training, and Experience

    • Five or more years of supportive services, social work, or relevant housing services
    • Strong proficiency in Microsoft Office Suite (Excel, PowerPoint, Outlook, Word) and Adobe Acrobat.
    • Strong knowledge of tenant rights, housing laws, and community resources.
    • Proven ability to manage a team and lead service initiatives in a housing context.
    • Excellent interpersonal and communication skills, with a strong emphasis on relationship-building.
    • Experience in budget management and resource allocation.
    • Ability to collaborate effectively with external agencies and organizations.
    • Crisis intervention and conflict resolution expertise.
    • Multilingual skills (depending on the population served).
