System Director Case Management

The System Director of Case Management develops strong relationships with facilities, corporate leaders, and post-acute partners to drive efficient care and discharge plans. to ensure seamless Care Transitions and Discharge Processes. The System Director of Case Management oversees the Case Management and Utilization Management departments, collaborating closely with physician leadership to shape strategic initiatives that align with organizational goals and regulatory standards. The System Director emphasizes optimizing patient throughput, improving Care Transitions, and advancing the Care Management strategy while ensuring education, compliance, and effective Resource Utilization. The System Director of Case Management offers a dynamic opportunity to influence patient care strategies and operational success across a multi-campus Healthcare System.

Qualifications: 

• Registered Nurse Required
• Bachelor’s Degree in Nursing required
• Multi-Campus System Director of Case Management in an Acute Hospital System required
• CCM or ACM preferred

Responsibilities:

• Current with best practices impacting clinical outcomes and reimbursement, making recommendations on new approaches to effective patient transitions of care
• Develops, supervises, and monitors departmental activities
• Represents department at institutional meetings, activities, and to external agencies and organizations
• Promotes operational effectiveness and efficiencies to improve financial performance
• Interacts professionally with all internal and external stakeholders
• Creates and maintains a positive and supportive work environment for all staff
• Supervises and coordinates the day-to-day delivery of patient care related to Case Management and Utilization Review
• Participates in the goal setting process, recommends policy and procedure and assists in evaluating programs
• Acts as a change agent, evaluating present systems and creating efficient, effective systems
• Demonstrates the professional role model
• Implements cost effective solutions
• Strong team approach to ensure collaboration across the Care Continuum
