Job Description
Build a professional one-on-one working relationship with TANF clients to aid in eliminating challenges that are hindrances to obtaining and retaining employment. This includes: assessment, individualized employment plan and coordination of services.
Conduct new customer orientation - Help to ensure that positive outcomes and goals are met as required by contract - Work with schools, DSS offices and staff and management to ensure appropriate documentation is completed and received.
Maintain organized database of clients and keep detailed documentation of all contacts with clients - Ensure participants follow through with planned activities as outlined in contract with funding agency.
Provide referrals to appropriate community service or social service agencies when needed.
Document all participant activities in case notes.
Create solutions to participant participation problems.
Assist in the preparation of project reports as needed.
Work collaboratively with the employers to identify which jobs are available, what qualifications are needed, and which workshops and trainings can best prepare jobseekers for these positions.
Participate in activities including training, capacity building, and professional development - Work with leadership to identify operational challenges and to design and implement process improvements.
Maintaining and updating participant tracker and attending weekly virtual meeting to discuss progress.
Respond to requests for information or complaints from clients and DSS and elevate as necessary.
May coordinate and direct family wellness activities as needed.
Complies with all programs, policies and procedures.
Maintains strict confidentiality of all privileged information.
