Key Responsibilities:
Conduct initial and ongoing assessments to determine client needs and strengths.
Develop and implement individualized case management plans to address medical, mental health, housing, employment, and social service needs.
Assist clients in accessing community resources, including healthcare, housing, legal assistance, employment, and educational services.
Coordinate care with therapists, medical providers, probation officers, and other stakeholders to ensure a holistic approach to recovery.
Advocate for clients to receive necessary services while promoting self-sufficiency and empowerment.
Monitor client progress, update service plans as needed, and provide crisis intervention when necessary.
Maintain accurate and timely documentation in compliance with state, federal, and facility guidelines.
Ensure compliance with Kentucky Medicaid and Targeted Case Management (TCM) regulations.
Participate in team meetings, case consultations, and staff training to enhance service coordination.
Qualifications:
Bachelor’s degree in social work, psychology, counseling, or a related field (Master’s preferred).
Must meet Kentucky state requirements for Targeted Case Management certification or be eligible to obtain certification.
Experience working with individuals with substance use disorders and co-occurring mental health conditions.
Knowledge of trauma-informed care, harm reduction, and recovery-oriented practices.
Strong understanding of community resources, Medicaid services, and social service systems in Kentucky.
Excellent communication, documentation, and organizational skills.
Ability to work independently and as part of a multidisciplinary team.
Valid Kentucky driver’s license and reliable transportation for client appointments.
CPR/BLS certification (or willingness to obtain).
Preferred Qualifications:
Certification or training in Motivational Interviewing, SBIRT, or other evidence-based interventions.
Experience with electronic health record (EHR) systems and Medicaid billing.
Familiarity with 12-step programs, peer support, and recovery-based services.
