We are a well-established home health agency, proudly serving our community since 1996, and we are now expanding into telehealth services to improve patient access and care coordination. We’re looking for a skilled LVN to lead the day-to-day operations of this innovative program while working closely with our experienced Registered Nurse (RN) supervisor.

What You’ll Do:

Oversee daily operations of the telehealth program, ensuring smooth patient interactions and efficient workflows.

Coordinate remote patient monitoring & follow-ups to provide timely interventions.

Collaborate with the RN & care team to ensure compliance with clinical standards.

Engage with patients and families to provide education, support, and guidance.

Utilize cutting-edge telehealth technology to improve patient outcomes.

Who We’re Looking For:

✔️ Licensed Vocational Nurse (LVN) in the state of Texas. ✔️ Experience in home health, telehealth, or care coordination is a plus but not required. ✔️ Strong communication and organizational skills to manage patient care remotely. ✔️ Passionate about technology and innovation in healthcare. ✔️ Comfortable working with EMR systems and virtual patient engagement tools.
