    You will be responsible for creating, planning and run trainings for clients and/or employees. This is an important role in providing the information to train and educate for various programs. Providing high-quality care to patients and making a positive impact on their lives. Gain a thorough understanding of the local, state, and federal healthcare guidelines specific to Community Health Workers, Enhanced Care Management and Community Support.

    Responsibilities:

    · Development and implementation of training content and continued education

    · Following company policies and procedures to assist the client with their specific needs.

    · Conduct thorough assessments for each individual client regarding the areas of the client’s needs, including health, psychological, social, and finances.

    · Create comprehensive and individualized care plans while collaborating with clients, their families, and other involved parties

    · Coordinate access to necessary services, resources, and support networks for clients.

    · Documenting all contacts with clients using specific databases and verbiage specific to the program.

    · Monitor and evaluate the effectiveness of the care plan, eliminating barriers, and adjusting as needed to ensure optimal client outcomes.

    · Assisting clients in overcoming barriers using motivational interviewing techniques

    · Maintain accurate and updated documentation of client interactions, progress, and service referrals.

    · Collaborate with interdisciplinary teams as needed, such as healthcare professionals, social workers, and community organizations, to ensure proper care for clients.

    · Participate in case conferences, meetings, and training sessions as needed or required.

    · Provide guidance and support to incoming community health workers, assessing when to provide training refresher as needed.

    · Staying current with CHW expectations and developing training as needed.

    Duties/Responsibilities: Education and Experience:

    High School Diploma or equivalent

    Medical terminology and/or background preferred

    Customer Service experience preferred

    Required Skills/Abilities:

    Experience working in a community-based setting for at least 1 to 2 years preferred. Attention to detail

    Basic computer skills required

    Good communication skills, such as listening well, and using language appropriately.

    Ability and willingness to provide emotional support, encouragement, and motivation to

    members

    Demonstrates excellent written and verbal communication skills and strong interpersonal skills

    Licensed driver with automobile insurance in accordance with the state

    Good driving record
