    • Perform Medicare Annual Wellness Visits and Well-Child Visits to support preventive care.
    • Provide real patient care in a home setting—not just rooming patients.
    • Help prevent disease instead of just reacting to symptoms.
    • Build meaningful relationships with patients instead of rushing through visits.

    Travel Requirements

    • Based in Contra Costa or San Joaquin County.
    • Travel throughout Northern California, focusing on areas with a high number of patients.
    • Expect to work in one dense area for two days at a time before moving to the next.
    • "Road warrior" mindset required —this role is for those who enjoy travel.
    • When traveling within your core region, a $0.55 per mile travel reimbursement is provided.
    • If traveling beyond your core region, car, air travel, meal stipends, tolls, and accommodations will be covered.

    What You Need to Apply

    • One or more years of experience as a Medical Assistant (CMA, RMA, or equivalent preferred).
    • At least six months in Internal or Family Medicine (or another primary care setting).
    • Experience performing Medicare Annual Wellness Visits and Well-Child Visits.
    • Comfort working in patients' homes (home health or in-home service experience is a plus).
    • Based in Contra Costa or San Joaquin County and willing to travel regularly for patient visits.
