Our Client is seeking a Travelling Advanced Practice Provider (APP)—open to Nurse Practitioners (NP) or Physician Assistants (PA)—to join their team, focusing on the Central Region. This role involves nearly 100% travel across states like Ohio, Michigan, Texas, Oklahoma, Georgia, and more.

    As part of the ACO program, you will provide compassionate and comprehensive care to Medicare patients, particularly those in underserved areas.     Your Role

    As a Travelling Advanced Practice Provider, you will:

    • Travel extensively to perform comprehensive care exams (CCE visits) and patient engagement visits.
    • Collaborate with multidisciplinary teams to develop individualized care plans.
    • Provide education and support to patients and their families to promote wellness.
    • Deliver care in private homes, nursing facilities, assisted living facilities, and other settings.
    • Maintain accurate and timely documentation in EMR systems.
    How You Qualify
    • Master’s degree in Nursing or Physician Assistant Studies.
    • Board Certification as a Nurse Practitioner (NP) or Physician Assistant (PA).
    • Active DEA license and CPR certification.
    • State licensure as an adult/geriatric Nurse Practitioner or Physician Assistant (or willingness to obtain multiple state licenses).
    • At least one year of experience as an APP; hospice or palliative care experience is preferred.
    • Proficiency with EMR systems such as PointClickCare and Chartpath.
