Travelling Advanced Practice Provider - ACO (Central Region)

As part of the ACO program, you will provide compassionate and comprehensive care to Medicare patients, particularly those in underserved areas. As a Travelling Advanced Practice Provider, you will:

• Travel extensively to perform comprehensive care exams (CCE visits) and patient engagement visits.
• Collaborate with multidisciplinary teams to develop individualized care plans.
• Provide education and support to patients and their families to promote wellness.
• Deliver care in private homes, nursing facilities, assisted living facilities, and other settings.
• Maintain accurate and timely documentation in EMR systems.

• Master’s degree in Nursing or Physician Assistant Studies.
• Board Certification as a Nurse Practitioner (NP) or Physician Assistant (PA).
• Active DEA license and CPR certification.
• State licensure as an adult/geriatric Nurse Practitioner or Physician Assistant (or willingness to obtain multiple state licenses).
• At least one year of experience as an APP; hospice or palliative care experience is preferred.
• Proficiency with EMR systems such as PointClickCare and Chartpath.
