Our Client is seeking a Travelling Advanced Practice Provider (APP)—open to Nurse Practitioners (NP) or Physician Assistants (PA)—to join their team. Your RoleAs a Travelling Advanced Practice Provider, you will:
- Travel extensively to perform comprehensive care exams (CCE visits) and patient engagement visits.
- Collaborate with multidisciplinary teams to develop individualized care plans.
- Provide education and support to patients and their families to promote wellness.
- Deliver care in private homes, nursing facilities, assisted living facilities, and other settings.
- Maintain accurate and timely documentation in EMR systems.
How You Qualify- Master’s degree in Nursing or Physician Assistant Studies.
- Board Certification as a Nurse Practitioner (NP) or Physician Assistant (PA).
- Active DEA license and CPR certification.
- State licensure as an adult/geriatric Nurse Practitioner or Physician Assistant (or willingness to obtain multiple state licenses).
- At least one year of experience as an APP; hospice or palliative care experience is preferred.
- Proficiency with EMR systems such as PointClickCare and Chartpath.
