EMPOWERING LIVES. RESTORING FAMILIES
The Clinician is responsible for providing clinical triage services for Children Requiring Assistance (CRA) and their families, referred primarily from the court, schools and DCF. Services include working with a Family Partner to conduct standardized intake, screening, assessment, service planning and referrals. The Clinician also works with the Family Partner to facilitate groups for youth and parents, such as Youth Anger Management and Parenting Skills & Support.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Provide screening intakes and clinical assessments (CANS) for CRA-related families.
Work collaboratively with the Family Partner through all steps of family engagement and assessment and support planning.
Provide family and group counseling; facilitate psycho-educational groups for program participants and their families.
Create Family Support Plans with willing families based on needs identified from intake, screening, and assessment.
Collaborate with the Family Partner, DCF, courts, school systems, attorneys, and other behavioral health/mental health providers on the best course of treatment for youth and their families and make referrals accordingly.
To adhere to program and agency policies and procedures
Notifies Program Director immediately in the event of a crisis, suspected illegal activity, or any behavior constituting an incident in the Program.
Performs client case management
To perform other duties as assigned by the Program Director or Clinical Director
Communicates with staff surrounding necessary client information
Serves as a resource in providing clinical support to staff as needed
Attends Clinical Team Meetings; Program Meetings; May fill in for other administrative team members at meetings as needed
Organizes and maintains all family client files and related information
Maintenance of program materials in adherence with state license and regulations
Seeks and uses management staff for training and supervision.
Attends Agency and Department required meetings and trainings.
Attends trainings and obtains CEUs in order to maintain professional licensure.
Follow up with families within 2 weeks of referral to see if referred services have connected with families. Work in collaboration with other FRC staff to ensure follow up occur 60 days later to ensure families’ satisfaction with the services of the FRC through a Family Satisfaction survey.
Review Family Support Plan with families at the end of 6 months from assessment completion to review adherence to the plan and/or identify additional necessary services and/or to adjust the plan if desired.
Maintain standards for clinical record keeping and monitor participant records for completeness and compliance, including data input into Gandara’s EMR system and the UMASS system for FRC specifically.
Provide training as needed on clinical topics to families and children/adolescents.
Provide outreach as needed to court personnel, police, school, and after school program personnel, community-based family agencies, health and behavioral health facilities and other entities to engage families.
