    We are seeking a full-time In-Home Therapy Services Therapist to provide community-based/in-home individual and family therapy for children, youth, adolescents and their families. The person in this position will work with clients and families/natural supports to assess their strengths and needs, assisting families with planning, developing, implementing, and assessing treatment and ancillary services, recommending the types of treatment modalities needed and providing these services directly or by referral. Qualifications: At a minimum, a Master’s Degree in counseling, social work, or psychology with a full or provisional license is required to perform this role (i.e LCMHC/A).

    We are seeking a full-time In-Home Therapy Services Therapist to provide the following services:

    The primary purpose of this position is to provide community-based/in-home individual and family therapy for children, youth, adolescents and their families. The person in this position will work with clients and families/natural supports to assess their strengths and needs, assisting families with planning, developing, implementing, and assessing treatment and ancillary services, recommending the types of treatment modalities needed and providing these services directly or by referral.

    • Serve as a member of the client’s clinical treatment team and create, develop, monitor, revise, and implement the client’s Person-Centered Plan.
    • Participate in the development of crisis plans and transition plans when clients move in and out of services.
    • Provide individual and family focused support to clients and their families/natural supports based on their needs, diagnoses and best evidence-based practices.
    • Provide any written documentation as required. This will include treatment notes, goals and plans for the medical record (e.g., Service notes, person-centered plan, etc.)
    • Implement evidence based practice selected by agency.
    • Work to implement therapeutic interventions, crisis interventions, behavioral interventions, psycho-educational activities and other best evidence-based practice therapies.
    • Work with clients, families, natural supports and collateral agencies in reviewing, applying and achieving goals as indicated in the Person-Centered Plan.
    • Contribute to and participate in child and family team meetings.
    • Provide parent/guardian/natural support training that will aid persons interacting with the client in dealing more effectively with the client.
    • Assist in all service delivery as formally planned with the clinical team.
    • Provide to parents/natural supports/guardians training that will help them better support the individual
    • Assist in accessing benefits and appropriate formal services

    Please note that this position is Community Based and relatable to providing Intensive In-Home services.
