MEAP Intake and Outreach Specialist

OVERVIEW:
The Care Coordinator is responsible for providing information, referral, advocacy, and case plan design for MEAP crisis and vulnerable customers.
KEY RESPONSIBILITIES:
o Connect daily with customers to develop and implement self-sufficiency plans.
o Design client specific resources to assist in removing barriers; Conduct follow-up after 6 months; Document and calculate all movement on the self-sufficiency scale.
o Identify and remove barriers to self-sufficiency; Design referrals to assist LSP customers with achieving their self-sustainable goals.
o Document all customer action steps and record all referrals.
o Provide validation support for agency submitted LSP applications.
o Conduct LSP coordination training for partner agency staff.
o In addition to the core responsibilities of the position, may be asked, or may seek, to engage unique abilities, strengths and passions through participation on cross-functional teams.

QUALIFICATIONS:
o Ability to handle stressful situations with calm and courteous demeanor
o Strong organizational, interviewing and interpersonal skills
o Ability to work with a diverse group of individuals from various social-economic status
o Knowledge of government, nonprofit, public health and/or human service resources
EDUCATION AND EXPERIENCE REQUIREMENTS:
o A high school diploma or associate's degree with some social services coursework
o A minimum of 1-3 years of experience in a call center or similar work environment
o Proficient with Microsoft Office applications and Internet usage.
