    Job Description

    We are seeking a detail-oriented and organized Scheduler to coordinate and schedule Nurse Practitioner visits for our Transitional Care company. The scheduler will be responsible for managing and optimizing the schedules of our Nurse Practitioners to ensure that patients receive timely , high-quality care. This role requires excellent communication and organizational skills, as well as the ability to work in a fast-paced environment.

    · Location: Orange County, CA

    · Work schedule: Monday through Friday, 8:30am – 5:00pm

    · Reports to: Chief Nursing Officer

    · Hourly Pay $18 - $25

    • Schedule home visits for nurse practitioners, ensuring timely and efficient coordination with patients and caregivers.
    • Communicate with nurses and patients to confirm appointment details, including time, location, and specific patient needs.
    • Adjust schedules as needed based on changes in patient needs, staff availability, or other unforeseen circumstances.
    • Maintain accurate and up-to-date scheduling records in our scheduling software and systems.
    • Collaborate with the clinical team to ensure all patient visits aligns with care plans and regulatory guidelines.
    • Provide excellent customer service to patients and families by addressing inquiries or concerns about visit schedules.
    • Communicate with the team about patients’ changing needs and ensure all visits are covered.
    • Conduct verbal patient satisfaction survey with patients at the end of their care.

  • Qualifications

    Qualifications

    • High school diploma or equivalent; additional education or training in healthcare administration, office management, or a related field is a plus.
    • Previous experience in scheduling, preferably within the healthcare or home health industry.
    • Strong organizational and time-management skills.
    • Proficiency in scheduling software, electronical medical records (EMR) systems, and basic computer applications (e.g., Microsoft Office Suite)
    • Excellent communication skills, both verbal and written.
    • Ability to work in a fast-paced environment and handle multiple tasks simultaneously.
    • Compassionate and patient-centered approach to service.
