    ABOUT WE CARE DAILY CLINIC: 

    We Care Daily Clinic (WCDC) is a warm, welcoming, and modern facility that provides treatment for addiction in a person-centered environment. WCDC seeks to renew hope and purpose for those battling opioid use by removing barriers, providing a holistic approach to recovery, and utilizing evidence-based treatment to restore healthy living. Our top priority is to provide the highest levels of patient-centered care to those we serve.   

    WCDC is a Medication Assisted Treatment (MAT) facility using FDA approved medication paired with behavioral health counseling. Our treatment plans are built around patients in a supportive, safe environment to create a personalized pathway to recovery. In addition to recovery, we also offer planned patient transportation and family and social supports.

    The Mobile Clinic Regional Outreach Coordinator will support the mission of the mobile clinics by engaging in patient outreach in the community, ensuring provision of resources, consistency of outreach driven services and increased census. The Mobile Clinic Regional Outreach Coordinator is responsible for ensuring that current and future patients of the mobile clinics have increased accessibility to services both on the mobile units and the brick-and-mortar clinic, communicating to Admissions Manager opportunities and successes of outreach efforts.

    This position is for an energetic professional who is willing to take on multiple tasks and has experience in handling a wide range of outreach and clinical support related tasks and can work independently with minimal supervision. This position requires a well-organized and flexible person who enjoys the complex challenges of supporting a diverse population in the community.

    RESPONSIBILITIES:

    • Communicate with Admissions Manager and PSR staff to ensure the effectiveness of mobile clinics with initial patient engagement and census development.
    • Develop an assertive system of street outreach specific to support mobile clinics in both Seattle and Tacoma by canvassing local encampments, shelters, etc.
    • Be an inviting presence in the mobile clinic for both patients receiving services and those inquiring about enrollment.
    • Educate new patients on the benefits of MAT for opioid use disorder and the process of receiving MAT services at both the mobile and stationary clinics.
    • Work with Transportation team in creating effective transportation routes based on the needs of patients participating on the mobile clinics or traveling to the clinic in Auburn.
    • Assist in maintaining schedule for prospective patients; work with Provider and PSR teams to ensure consistency and availability.
    • Verify patients are enrolled in Medicaid via P1 and are eligible for services.
    • Refer patients to PSR once verified for full registration; work with PSR team to register, as needed.
    • Work within local jails and shelters to create system of referrals to mobile clinics.
    • Manage inter-agency transfers between mobile clinic and brick and mortar clinic.
    • Work with Admissions Manager to identify opportunities within the community as seen through street outreach efforts.
    • Partner with Peer team to create support network within We Care; train on mobile clinic outreach, as needed.
    • Maintain thorough case management notes when providing wrap around services and resources.
    • Coordinate with clinical team regarding referrals for IIP, IOP or OP programming, as requested.
    • Participate in daily huddles to discuss mobile clinic planning and operations
    • Other duties as assigned

     

    POSITION REQUIREMENTS

     

    EDUCATION & CERTIFICATIONS:

    • Bachelor’s Degree, preferred
    • Valid driver’s license
    • Must possess or have the ability to possess and maintain First Aid and CPR Certification

    EXPERIENCE:

    •     Minimum three (3) years of outreach experience required

    •     Basic knowledge of computers and programs to include MS Office and basic CRMs.

    •     Experience working in both a clinical and outreach setting

    •     Proven community resource knowledge in King and Pierce counties

    •     Knowledge of MAT procedures and release protocols in local jails

    •     Extensive experience with crisis management and de-escalation

    •     Experience and ease in working with a diverse and challenging community

    •     Experience with effective case notation and clinical writing

    •     Able to work effectively both individually or in a team setting

    Or an equivalent combination of education, training, and experience.

    ADDITIONAL:

    • Ability to manage your time efficiently.

    • Availability to work nights and weekends as required.

    • Work well when supervisors are not present.

    • Candidates shall be subject to all applicable regulations; alcohol testing and employment will be contingent on the results of said testing.

    • Must pass a criminal background check, a drug screen and meet all health and immunization requirements required by federal, state or Tribal laws and regulations.  

    • Must successfully pass a reference check with previous employers.

    • Ability to sit or drive for a minimum of an eight-hour workday

    • Ability to lift 10-50 lbs.

    • Ability to carry 10- 50 lbs.

    • Ability to work outside for extended periods of time
