About the Role:

We are seeking a Weekend On-Call Home Health/Hospice Admission Nurse to join our team at Harmony Home Health & Hospice in Murray, UT. This role offers the opportunity to make a difference in the lives of patients and their families, while working with a supportive and dedicated team.

Responsibilities:

Conduct home health and hospice admissions on weekends

Assess patient needs and develop individualized care plans

Coordinate with interdisciplinary team to provide comprehensive care

Communicate effectively with patients and their families

Ensure compliance with all regulations and company policies

Requirements:

Current RN license in the state of Utah

Minimum of 2 years of nursing experience, preferably in home health or hospice

Strong clinical assessment and critical thinking skills

Compassionate and empathetic demeanor

Ability to work independently and as part of a team
