    About Puentes de Salud

    Puentes de Salud (Bridges to Health) is a 501(c)3 nonprofit organization that promotes the health and wellness of Philadelphia’s rapidly growing Latine immigrant population through high-quality health care, innovative educational programs, and community building. The foundation of Puentes de Salud rests on the belief that community health is not the sole domain of health care providers. Health and wellness result from a collaborative, multidisciplinary effort that focuses on addressing the conditions in which we grow, learn, live, work and age.

    Position Overview

    The Wellness Supervisor will play a key role in supporting Puentes de Salud’s Health Division by overseeing Wellness programs, particularly the Food Access program, and ensuring seamless service delivery for our community. Reporting to the Health & Wellness Director, this position involves collaboration across multiple departments and partners to enhance the health and wellness of the Latine immigrant population in Philadelphia.

    Key Responsibilities

    Program Support and Coordination

    Food Access Program: Assist in the implementation and management of the Food Access program, ensuring efficient service delivery and client engagement. Work closely with the Food Pharmacy Coordinator to coordinate food distribution, manage partnerships, and oversee food access initiatives.

    Food Satellite Distribution: Supervise the Food Access Driver and manage the Food Satellite Distribution program, ensuring access to healthy food for Latine communities. Maintain strong communication with partner organizations and assist with logistical coordination.

    Food Pharmacy Operations: Staff the Food Pharmacy on Thursdays (10:30 AM to 3:00 PM) to support direct service delivery, ensuring that clients receive the appropriate resources and assistance.

    Community Partnerships: Cultivate and maintain partnerships with key food access organizations such as Old Pine Community Center, Sharing Excess, Philabundance, and the Make Well Known Foundation. Represent Puentes de Salud at meetings and work collaboratively to address food security challenges.

    Access to Food Committee: Lead monthly Access to Food Committee meetings to assess ongoing food access needs and strategize solutions.

    Administrative Support

    Timesheet Management: Process timesheets for wellness contractors on a weekly, biweekly, and monthly basis, depending on the role.

    Volunteer & Intern Coordination: Assist in recruiting, training, and managing volunteers and interns to support Wellness programs and initiatives.

    Event Planning: Coordinate Puentes de Salud’s presence at community outreach events, including tabling for resource sharing and health education.

    Community Outreach: create and disseminate resources and information through Puentes’ various communication channels, ensuring accessibility and engagement.

    Data Collection & Reporting: Support the collection and analysis of wellness data, preparing quarterly reports for the Health & Wellness Director to assess program effectiveness and community impact.

    Collaboration & Health Initiatives

    Liaison Role: Facilitate communication and collaboration between the Clinic and Wellness teams, ensuring a seamless continuum of care for Puentes clients.

    Health Initiatives Coordination: Assist in coordinating key health initiatives such as mobile mammogram screening clinics, ensuring effective planning and execution.

    Schedule & Work Environment

    Part-time: Up to 20 hours per week

    Work Location: 50% remote, 50% on-site at Puentes de Salud’s Community Wellness Center

    Schedule: Some evening and weekend hours required; flexibility with work schedule and environment is essential

    Qualifications

    Education & Experience: Bachelor’s Degree and 2-3 years of relevant experience, or equivalent combination of education and experience. Experience working with immigrant populations and in program coordination is essential.

    Language Skills: Fluency in both oral and written Spanish and English is required.

    Technical Skills: Proficiency in Microsoft Office suite (Word, Excel, PowerPoint) is required.

    Communication & Organizational Skills: Strong bilingual (English and Spanish) verbal and written communication skills, with the ability to interact effectively with clients, staff, and community partners. Strong organizational, planning, and problem-solving skills are preferred.

    Additional Requirements: Valid driver’s license and access to a personal vehicle with current registration and insurance is preferred.

    Physical Demands

    Ability to lift up to 40 lbs regularly

    Visual: Regular computer use and email communication

    Hearing: Use of phone and communication with clients and partners

    Working Conditions

    This position operates in an office environment free from significant noise or temperature extremes but may involve occasional work in environments without elevators.
