Job Duties
• Develops and maintains a clinical practice by providing patient care service and cooperating with medical and nursing personnel to optimize the pharmacotherapeutic aspects of patient care. Provides optimal selection of pharmacologic agents, designing a regimen to maximize therapeutics benefit and minimize toxicity. Provides daily follow-up on the clinical effects of the regimen with adjustments as necessary to achieve desired outcomes.
• Verifies computerized medication orders accurately, makes appropriate changes or additions to the patient’s therapy or treatment plans when necessary, verifies compounded medications via dispense prep.
• Provides drug and clinical information to other healthcare professionals in response to specific questions on newly evolving issues related to the care of infusion center patients. Provides professional education regarding new drugs, biologics, or technologies for specialized drug delivery. Provides a significant level of education to professionals, trainees, and patients. Responsible for monitoring and assessing performance of the learner and providing feedback using appropriate learning objectives that are aligned with the desired learning outcomes.
• Supervises and trains pharmacy technicians.
• Orders medications and supplies and maintains inventory of the infusion center pharmacy.
• Ensures compliance with patient, quality, and USP standards and maintains quality assurance documentation.
• Designs, supports, and implements policies to provide effective and efficient use of resources ensuring that budget, quality, and value targets are met in collaboration with the Director of Pharmacy.
• Develop, review, assist, and maintain, in collaboration with members of the Department of Hematology/Oncology, various protocols and regimens, participating in testing of therapy and treatment plans build within the electronic health record.
• Participates in infusion related committees and department staff huddles.
QUALIFICATIONS/BASIC JOB REQUIREMENTS
Education:
• Pharmacy degree from an accredited college of Pharmacy
Experience/Skills
• Clinical pharmacy experience and residency is preferred.
• Two years of experience in hospital or infusion center pharmacy preferred. Person should be educated regarding IV admixture, drugs, disease states, pharmacology, and infection control and pharmacy law.
• Needs to have excellent verbal and written communication skills, as well as an ability to work all IV room equipment, compound sterile products, and train others.
• Requires expertise in aseptic techniques and hospital pharmacy policy and procedure.
• Computer Skills: proficient in software (MS Outlook, Word, Excel), comfortable with electronic health record (Epic), and embraces technological change
LICENSE/CERTIFICATION
• Current license by the State of Illinois Department of Professional Regulation (of Education and Registration) as Registered Pharmacist.
• Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited Doctor of Pharmacy degree program
• PGY1 and PGY2 (in the area of specialization) Residency or the equivalent experience preferred.
• Board Certification preferred.
